To schedule an appointment please call (305) 461-1700. While we appreciate
scheduling ahead when possible, we also realize the importance of accommodating
"last minute" requests. When scheduling please fax a copy of your insurance
card (front and back) and your prescription in order to assure the right test is
scheduled and to verify the insurance ahead of time. The fax number is 305-461-1716.
One-way or round-trip transportation of patients, both ambulatory and
wheel chair, is provided free of charge in Miami Dade county. Please inform
receptionist if transportation is required at time of scheduling.

Once the test is scheduled, we will be contacting
patients with special instructions regarding their particular
exam 1-2 days in advance.

Parking for the facility consists of parking meters located in front,
on side, and in back of the building. Handicapped spaces are located
in front and in back of the building. Parking meters in front and on side
of facility begin operations at 9AM and accept no more than 2 hours at a time.
Parking meters behind the building are operational 24 hours a day and accept
no more than 3 hours at a time. If your meter is set to expire and you are not
yet finished with your study, you may give the change to the receptionist and we
will feed the meter for you.
